STEP 2: PRINT YOUR EMERGENCY CARD, OTC MEDICATION FORM, AND HEALTH INVENTORY FORM
DOWNLOAD - HEALTH INVENTORY FORM (ENGLISH)
DOWNLOAD - HEALTH INVENTORY FORM (SPANISH)
STEP 3: COME TO YOUR STUDENT’S ASSIGNED REGISTRATION DATE/TIME
Bring the following items:
